Health Records Department
The Health Information (Medical Records) Department at The Orthopaedic Group can be reached by telephone from 10:00 a.m. - 1:00 p.m., Monday thru Friday. Please call us during these hours at (203) 401-4628. Restricting calls to these times allows us to process patient requests and complete disability forms in a more timely manner. Requesting Medical RecordsPlease note that we provide two different authorization forms: 1.) Request to Access Protected Health Information. This authorization is used solely for patients requesting to access his or her protected health information (medical records). Please print a copy of our HIPAA compliant authorization, fill it out completely. Then return it to the Health Information Dept. by fax at (203) 401-0035 or mail it to TOG, Health Information Dept., 199 Whitney Avenue, New Haven, CT 06511. We will process your request and mail records back to you within 30 days of receipt. It is our goal to process all requests in a timely manner. 2.) Authorization for Use and Disclosure of Protected Health Information. This grants the release of the patient's protected health information to another party, i.e. an attorney, physician, disability insurance company, etc. Print the form, then fill it out, sign and date it. Then return it to the Health Information Dept. by fax at (203) 401-0035 or mail it to TOG, Health Information Dept., 199 Whitney Avenue, New Haven, CT 06511. Disability and Benefit FormsThe Health Information Department also handles the processing of disability forms, FMLA forms, S.S. benefit forms, etc. We require a fee of $15.00 to complete a form (with the exception of a workers compensation forms). A completed copy of our Authorization for Use and Disclosure of Protected Health Information must be included. All forms can be mailed to our address along with a completed authorization and a check. Once received, we will process the form and mail it back to the patient or directly to the company requesting the information within one week. We understand that it can be very stressful to take care of your health needs and also take care of the paper work that may be involved.
