The Medical Records Department is staffed with a group of employees dedicated to help and serve you. We understand that it can be very stressful to take care of your health needs and also take care of the paper work that may be involved. A written request and a completed authorization is required for the release of a patient’s medical record. (Please see section below for instructions on how to print out these forms).
All patient records requests MUST have a completed and signed HIPPA compliant authorization BEFORE a request can be processed. All requests will be processed within 30 days of receipt. A fee of .65 cents per page may be charged for copies of Medical Records.
Our HIPAA Notice of Privacy Practices describes how medical information about you is used and disclosed, and also how you can get access to that information.

How to Request Medical Records
- Request to Access Protected Health Information. Please click on this link to print out a copy of our HIPAA (Health Insurance Portability and Accountability Act) compliant authorization. This authorization is used solely for patients requesting to access their own protected health information. Fill it out completely and return it to the Medical Records Dept. by fax to (203) 401-0035 or mail it to us at: The Orthopaedic Group, Medical Records Department, 199 Whitney Avenue, New Haven, CT 06511.
- Authorization for Use and Disclosure of Protected Health Information grants the release of the patient’s protected health information to another party, i.e.: an attorney, physician, disability insurance company, etc. Please click on this link to print out a copy of our form, fill it out, sign and date it and return to us either by fax or by mail in the numbers and mailing address provided above.
Disability and Benefit Forms
We can process Disability and Benefit forms that are related to your orthopedic treatment and care. There is a $15 processing fee (exceptions: Social Security forms and Workers Compensation forms)
- Family Medical Leave Act (FMLA) forms
- Social Security Forms (S.S. benefits)
- Workers Compensation forms
- Aflac forms
- Insurance disability forms
A completed copy of our Authorization for Use and Disclosure of Protected Health Information must be included. All forms can be mailed to the above address along with a completed authorization and the $15 processing fee. Once received, we will process the form, and mail it back to the patient or directly to the company requesting the information within one week.
Copies of your X-rays
These can be obtained by contacting The Orthopaedic Group, Radiology Department at (203) 865-6784. If you had digital x-rays there will be a processing fee of $10 to copy these films onto a disc.
