For Doctors Referring Patients:

If you would like to send your patient to see one of our physicians, print out our TOG Consultation Request Form, fill it out, then fax it to our Scheduling Department at (203) 865-6788.

Our team of dedicated appointment schedulers will make an appointment that is convenient for your patient. We will then fax your office a confirmation so you'll know when your patient is scheduled to be seen.

For New Patients Coming In for an Appointment:

Patients can now complete paperwork prior to their scheduled appointment. Just print out the forms below, fill them out & bring to the appointment:
